Please complete this form to submit your announcement(s) to the church office two (2) weeks prior to the date that you wish for the announcement to be announced. Please note that all announcements are subject to editing and the office will determine placement of your information (i.e. church website, newsletter, AV, Social Media etc.).
Van Use Request
This form is used for members (on behalf of their ministry) to request use of the Mt. Zion Church Van to transport people to/from church/church events, community events, pick up those attending events at Mt. Zion and the like. It is requested that this form be submitted at least 5 (five) days prior to the requested date. The driver of the MZBC van/bus must hold a valid Driver's License and be on the church approved driver list. For insurance purposes, please list the person who will be driving the vehicle.
